Refer A Friend Rules

For the patient referring a friend

• In order to receive your credit, the referred friend must complete all requirements as outlined in the New Referred Friend Requirements section. 

• You must be currently enrolled Blueprint Health Patient.

• Your membership must be in good standing.

• Your credit will be posted to subscription payment following when your referred friend meets their eligibility requirements.

• Feel free to refer as many friends as you wish into the program.  We will credit you for each successful referral as outlined in the New Referred Patient Requirements section.

For the newly referred patient

•  In order to receive the credit for being referred to our program, you must be a new patient.  A new patient is considered a person who has not received service from Blueprint Health for the past 12 months.

• You must complete the initial lab process, review and subscribe to our service.

• The credit will be applied within 30 days of meeting the previous requirements.

• Your credit will be applied towards your subscription fee.
