Refer A Friend Rules

For the patient referring a friend

• In order to receive your credit, the referred friend must complete all requirements as outlined in the New Referred Friend Requirements section. 

• You must be currently enrolled Blueprint Health Patient.

• Your credit will be credited to the next order following when your referred friend meets their eligibility requirements.

• Feel free to refer as many friends as you wish into the program.  We will credit you for each successful referral.

For the newly referred patient

• In order to receive the credit for being referred to our program, you must be a new patient.  A new patient is considered a person who has not received service from Blueprint Health for the past 12 months.

• You must complete the initial lab process, review and start a medication therapy program from Blueprint Health.

• The discount will be applied to your first order.
